We participate with most insurance plans and will submit the claim on your behalf. Please note that your bill is your responsibility whether your insurance covers your services or not. Patients are encouraged to check benefits and required referral with their insurance carrier prior to appointment. We do not guarantee coverage or in-network status.
If you do not have health insurance, we are still happy to see you. We offer a simple self-pay fee schedule available upon request.
* Based on your insurance plan and deductible, payment for your entire visit and/or in-office procedures may be required at time of service. Photo ID and valid insurance card required at every visit

If you do not have insurance or we don’t participate with your plan, no worries ! We offer convenient transparent pricing for all services upon request. All cosmetic procedures are required to schedule a consultation with Dr. Mays for proper assessment and recommendations. The consultation fee is applied to any procedure booked same day.
